At the start of services, we will conduct an evaluation on the patient for presence of significant social and emotional factors related to their underlying health problems. We work with families and the patients themselves to promote continuity of care. We handle health related issues and assist the patient in coping with lifestyle changes through:

• Individual or group conferences
• Restoration of optimal social standing and health adjustments
• Utilization of community resources
• Resumption of life and learning to live with disability
